Constructed as a H-34A-SI and delivered to the US Army in January 1958 as 57-1684. Converted to a H-34C in January 1960, by February 1960 it was designated as a VH-34C. Transferred to the Military Aircraft Storage and Disposal Center (MASDC) with inventory number 3H0268 in October 1972. Loaned to Pima by the National Naval Aviation Museum. By 1991 the pictured marking were applied. Photographed in March 2020 with the paint completely stripped for restoration.
