Delivered new (as PI-C803) 27th Jan 1968. On the 18th Apr 1977 flight PR421 RJTT-RPLL was 1000m into take-off when it lifted off early. The aircraft banked left, touched down again, veered off the runway and continued 800m before coming to rest with the undercarriage and all 4 engines torn off.The left elevator geared tab drive arm assemblies and elevator gust lock crank assembly had failed.
