Delivered new 27th July 1979. On the 13th June 1996 the aircraft crashed on take off from RJFF (Fukuoka) after no 3 engine had a fan blade separation. The crew opted to abort after V1 which resulted in the aircraft skidding 620m past the end of the runway threshold.
