Crashed near Packenham, Victoria, on January 4, 1987, after the tailplane was torn off by the accidental deployment of the reserve parachute of a skydiver. The aircraft plunged vertically into the ground but not before the pilot was able to parachute to safety. The skydiver was knocked unconscious but came to in time to deploy her main chute.
