Delivered new in 1978 to TAVINA as HK-2121 and here, after several ownership changes, operated by Superior Aviation of Wisconsin on courier services. On 9 November 2000 it was destroyed after take-off from Fort Wayne/IA in a collision with terrain after the pilot lost situational awareness in IMC, due to both attitude indicators loosing power.
