Built in 1965, imported for the third time in 1997 and here wearing Canadian Helicopters colors, but devoid of titles since it is on its way to become N612AZ with Carson Helicopters. On 5 August 2008 it was destroyed in a take-off crash from 6000 ft near Weaverville/CA while at gross weight.
