On 30 January 1978 this aircraft was observed to be on fire as it took off from Bergstrom AFB, TX. The crew reported a fire warning light and began dumping fuel. Fire lights on both engines and un-commanded pitch transients were reported. Another RF-4C joined up to provide chase and confirmed the aircraft was on fire. The crew safely ejected 13 minutes after take-off and the aircraft came down 4 miles NNE of Lockhart, TX.
