The "City of Santo Domingo" (built 1956, ex USAF 54-0173) ditched off San Juan on April 5, 1990. Was on 3-engine ferry to SDQ with #2 engine shut-down, when incurred #3 engine fire warning during climb, followed by improper response resulting in continued fire on #3 engine and oil-starvation induced failure of #1 engine.
