Delivered to USN in November 1968. Assigned to VAL-4 in Vietnam from January 1969 through April 1972. Later transferred to the USMC and converted to OV-10D configuration. The aircraft is marked as when assigned to VAL-4. On 25 May 1969, while flying this a/c, LT. Pete Russell & LT(jg) Jeff Johnson were strafing a target when the a/c was hit by enemy fire. LT. Russell was fatally injured and LT(jg) Johnson flew the a/c back to Binh Thuy, South Vietnam, their base.
