Crashed at Wanganui River on December 22, 2004. The helicopter was carrying three kayakers with their kayaks and paddles in a net slung underneath. The light load swung up and hit the tail rotor. The pilot effected a safe but heavy autorotation landing. However HHT was burnt out after the occupants escaped.
