This was the last production F-4B, entering service with VF-121 at NAS Miramar, CA in January 1967. By May 1972, deployed aboard USS Midway with VF-161 and was quickly credited with shooting down a MiG-19 over North Vietnam. Around 1973 it was upgraded to F-4N status and deployed aboard USS Coral Sea with VF-111. Later transferred to VMFA-314 and VF-154.
