At the time operated by 29 Sq. Delivered to the RAF as XV421 in 1969. Crashed into the sea off the Falkland Islands in 1991. Both crew died.

This aircraft crashed into the sea north of McBrides Head, Falkland Islands while operating with 1435 Flight on October 30th 1991. Both crew were lost.

On 30 October 1991, while serving with 1435Flt out of RAF Stanley, Falkland Islands, control was lost during ACM with another Phantom. It disappeared into cloud and was not seen again. A five-day search recovered only small amounts of wreckage in the South Atlantic north of McBrides Head, East Falkland. (Approx. 50 miles north of Port Stanley). It was concluded that the crew became disorientated while in cloud, neither ejected and both were killed

Lost on a mission out of Port Stanley, Falkland Islands on October 31, 1991. It is believed the pilot became disorientated in cloud at night. Only small amounts of wreckage were found.
