Delivered to Okanagan Helicopters in 1980, this A350D was converted to an AS350B circa 1990 with Canadian Helicopters. It was written off in an accident on Birch Mountain near the British Columbia border with the Yukon on August 27, 1993. Little details are in the accident report, but the two people on board were rescued with minor injuries by another company helicopter.
