This aircraft was carrying out radio calibration training at its Coventry base on 17th August 2008, and whilst making an ILS approach to Runway 23 it collided with Rand KR-2 G-BOLZ making a visual approach. Tragically all four occupants of G-EYES and the single occupant of G-BOLZ received fatal injuries.

On 31st July 2020 this aircraft was about to touch down at Cotswold (Kemble) Airport when the left wing dropped and touched the runway. The pilot attempted a go-around, but the aircraft landed to the side of the runway and travelled across the grass before colliding with a parked aircraft. No injuries were sustained, but both aircraft were substantially damaged and it was cancelled from the USCAR in January 2021. The other aircraft involved was Twin Comanche G-ATXD.
