Originally US Navy serial 157858 and one of only eight utility versions of the UH-1E built. After construction in 1969 it served in the Vietnam war and was retired to MASDC in 1983. Here it is on the way to a forest fire engagement in Canada, according to the pilot. While heli-logging on 19 September 1998 near Price/UT, it went out of control and crashed when tail rotor effectiveness was lost due to the density altitude being nearly 11,000 feet,
