Providence Health Care merged with Empire Health Services in 1994 and was re-named Inland Northwest Health Services. The respective air ambulance services of the companies were merged to form Northwest MedStar. Headquartered in Spokane, W, Northwest MedStar served eastern Washington, northern Idaho, western Montana and eastern Oregon. Delivered new in 1999, N312MS joined Oregon based Life Flight Network which bought Northwest MedStar in 2016. The helicopter was re-registered N841LF in 2018 and is current at the time of uploading.

Originally registered N312MS flying for Northwest MedStar as part of the Inland Northwest Health Services (INHS). INHS was purchased by Lifeflight Network in 2016 and this EC-135 was re-registered N841LF in July 2018. Still flying for Northwest MedStar at the time of uploading, the helicopter carries only Lifeflight Network titles.
