Crashed into the Pacific Ocean 400 miles off the coast of Acapulco Mexico while operating from the Coast Guard Cutter Rush in 1987. All 4 crew members survived, but it sank and was left on the ocean floor.

A few months later on 10 January 1987, still assigned to CGAS Port Angeles, near Seattle, the helicopter ditched 400 miles off the coast of Mexico while operating from the cutter USCGC Rush, due to engine failure. Aircraft salvaged but written off. Photo from: San Diego Air & Space Museum
