This MiG-21F-13 was built by Aero-Vodochody as S106. It entered service with the Czechoslovak Air Force in November 1971 and was only officially wfu by March 1991 although it had already been placed in storage at Prerov by September 1990 (when I saw it there, lined up among dozens of other MiG-21s). It first went to the museum at Kbely in 1994 and one year later it went to the USA.

This is one of the 194 MiG-21F-13 examples built in Czechoslovakia under licence and is easily idenified as such by the absence of a windshield behind the pilot's head. This plane has been on display at the March Field Air Museum since 2002.
