During deployment with the 389th TFS to Da Nang AB, South Vietnam, this Phantom was damaged by an SA-2 Surface to air missile. After repair, it shot down a Mi-21 over North Vietnam on May 20, 1967. Pictured with the Oregon Air National Guard, the aircraft was retired October 1989 as the 123rd Fighter Interceptor Squadron re-equipped with the F-15A Eagle. It was loaned to the Frontiers of Flight Museum in Dallas, TX a few weeks later before transfer to the Cavanaugh Flight Museum during 1994.

In use as a BDRT aircraft at Fairford for a number of years, In 1987 it graced the static park, aircraft is now preserved at the Midland Air Museum Coventry Airport but in the SE Asia sand/green/tan camouflage paint scheme. Scanned from slide
