USAF serial 63-12691. Delivered to the Italian Air Force as MM54232. Modified to a TF-104G-M. Withdrawn from use at Grazzanise in 2003. Last noted in 2009. Later scrapped.

This Starfighter was built by Lockheed as TF-104G and received USAF s/n 63-12691. It was delivered in 1974 to the Aeonautica Militare and served 20° Grupo from the beginning. The dual riocket with wings was converted to -M (Modificato) standard by 4°RMV at Grosseto. It arrived from Grosseto (around July 1999) and was re-delivered after conversion back to Grosseto (around May 2000). It again served the 20° and was eventually withdrawn from use and was parked on a storage compound near maintenance area at Grazzanise from mid November 2013.
