On 30 September 2016, the aircraft unintentionally impacted the water off Lopez Island, Washington, while the pilot was attempting to land in poor visibility due to ground fog. The aircraft came to rest upright, but was substantially damaged and began shipping water. The pilot and three passengers successfully evacuated and were shortly rescued from the sea by boat. The aircraft subsequently sank and was not recovered.
