There are two versions as to when and where A16-10 ended on its nose. One says it was an accident at RAAF Richmond on October 31, 1940. The other says the date of the accident was December 31, 1940 and at Darwin. At the time of both accidents the aircraft was with 13 Squadron which was a Darwin-based bomber squadron. The photo is dated 1941. A16-10 was repaired by Clyde Engineering, a large engineering company, at its aircraft repair facility in Sydney. Back in RAAF service, A16-10 was sent to 1 OTU in Victoria.
