Leased from Heli Air Services. Operated Luqa Airport - Gozo Heliport service. Went down in the Indian Ocean on 8 January 1994 near Ari Atoll in the Maldives while on approach to land on the island of Kandholhudu. Both pilots survived but cabin attendant and eight passengers died. Manufactured in 1989, had logged 1225 hours and was on lease to Hummingbird Helicopters.

First helicopter used for Malta - Gozo services. Malta Air Charter lease; operated on a one-month trial period from 27 June 1990 to 27 July 1990, making its last trip to Gozo and departing on the same day (27 July 1990). Crashed into sea 7 miles south of Gorgona Island, Italy after distress call on 26 October 1992. Departed from Tarquinia, en route to Borgo Sesia. Two occupants both killed. At the time was in service with Free Air.
